Fiv with sperm donor: process and results

In vitro fertilization (IVF) is preferably carried out with the ovules and sperm of the future parents. However, when there is no male partner or when it presents serious fertility problems, semen donation is used.

Thus, the sperm used to fertilize the ova of the future mother will come from donors who have decided to give their gametes. The donation of semen in Spain is anonymous and altruistic so donors and recipients will not get to know each other.

Below you have an index with all the points that we will discuss in this article.

IVF with sperm donor: process and results

Process

To make an IVF, it is necessary to obtain the ovules on the one hand and the sperm on the other to later fuse them in the laboratory.

Thus, the patient must undergo a hormonal treatment of ovarian stimulation that will allow obtaining a greater number of ovules in the follicular puncture. This is the surgical procedure in which fluid is aspirated from the ovarian follicles (structures where ovules mature in the ovary). Subsequently, the follicular fluid is analyzed in the laboratory in search of the ovules.

As for semen, it comes from a donor bank. All donor samples are frozen after obtaining them for a minimum of 6 months to ensure the absence of transmissible infections.

IVF process with sperm donation

Once thawed, the sample is processed and used to fertilize the ovule, either conventionally or through ICSI (intracytoplasmic sperm injection).

Next, the fertilized ovules, already considered embryos, are left in culture until the day of the transfer. This can be done on day 2-3 or on day 5-6, depending on the characteristics of the situation and the way of working of each laboratory.

Fertilization and transfer

To favor the implantation of the transferred embryos, the patient receives estrogen and progesterone administered orally, vaginally or in patches. This hormonal medication will allow your endometrium to acquire the trilaminar appearance and optimal thickness for embryo implantation.

Two weeks after the transfer, the measurement of betahCG (pregnancy test) will be carried out, which will confirm whether the technique has been successful.

When to do an IVF with a sperm donor?

When the couple's sperm can not be available or the partner does not allow pregnancy, a semen donor is used.

Sperm donors are young and healthy children who undergo a series of physical and psychological tests that confirm their ability to be donors.

On the other hand, it is necessary that your seminal sample has an excellent seminal quality, with a high concentration and mobility of the sperm. In addition, since all the samples are going to be frozen, it is essential that the sperm survive the thawing test, that is, that they survive sperm freezing.

When is donor semen needed?

The main reasons why a semen donation is used are the following:

  • Severe male infertility: azoospermia, cryptozoospermia, oligozoospermia, teratozoospermia ...
  • Risk of transmitting hereditary diseases on the part of man
  • Lesbian couples
  • Single mothers

Success of IVF with donor sperm

The effectiveness of in vitro fertilization with donor semen depends to a large extent on the age of the woman, since the semen is of very good quality.

Maternal age is one of the limiting factors when talking about the success of IVF, because the female reproductive capacity decreases with increasing age, with the decline especially marked after 35 years.

The qualities of the uterus and the conditions of embryonic culture are other aspects to consider.

According to data from the SEF (Spanish Fertility Society), the gestation rate per transfer in cases of IVF with donor semen is slightly higher than 42%, while the delivery rate for each transfer is 26.2%.

These data are taken regardless of the origin of the ovules, that is, they do not distinguish whether the ovules come from the patient or the ovodonant.

How much does sperm donation cost?

The price of IVF with donor semen has an approximate cost of between 3500 and 5500 ?. The variation of the cost will depend on several factors such as the clinic where the process will take place, the needs of each situation, the number of attempts, etc.

Donor semen slightly increases the cost of assisted reproduction treatment due to the expenses derived from obtaining sperm, such as the financial compensation received by the donor, the medical tests that are done, the analysis of the seminal sample and the sperm freezing procedure.

The expenses related to the sperm donor are lower compared to the donation of ovules since the man does not have to take hormonal medication nor is a surgical intervention necessary to obtain the sample.

Questions from users

How is the sperm donor chosen?

The donor of semen is chosen based on the genetic compatibility and the similarity of physical qualities with the recipient. You can get more information here: selection of the semen donor.

What are the requirements that donors must meet to donate their semen?

They must be between 18 and 40 years old, completely healthy and with no family history of genetic diseases. In addition, your seminal sample must have excellent quality, above the reference values ??set by WHO.

What is the financial compensation received by sperm donors?

In Spain, donors receive between 30 and 40 euros for each semen sample deposited in the clinic or sperm bank.

Is there a change in the woman's treatment for using donor semen?

No, the hormonal treatment of ovarian stimulation and follicular puncture do not show variation between IVF with sperm of the couple and IVF with donor sperm. The process changes if donor eggs are used instead of ovules, but as for semen, everything related to the woman (future mother) is the same.
