Ovarian hyperstimulation syndrome: what is it and how is it cured?

The ovarian hyperstimulation syndrome (OHSS) is an abnormally high response of the ovary to the medication that is administered in assisted reproduction treatments, especially in in vitro fertilization (IVF). It will be aggravated if pregnancy is achieved in that cycle.

What is it and when does the SHO appear?

This alteration is triggered in the luteal phase of the controlled ovarian stimulation cycles, after the administration of hCG (human chorionic gonadotropin). Only in the presence of this hormone does the excessive response that causes this syndrome occur.

It appears more frequently when in vitro fertilization (IVF) treatments are performed, compared to artificial insemination (AI), because the hormone doses administered in an IVF are higher than for an AI.

In an IVF treatment, the patient is given hormonal medication (gonadotropins) to stimulate the development of more follicles. Thus, we get more than one ovule to mature, which is what happens monthly in a natural cycle, under physiological conditions.

Causes of SHO

When this medication stimulates the ovary more sharply than expected, it may happen that the size of the ovaries will grow excessively and, after ovulation (release of the ovules), the escape of part of the fluid from the ovary to the area of the belly and chest.

The acute extravasation of fluids out of the bloodstream due to increased permeability of blood vessels can compromise the function of the hepatic, hematological, renal and respiratory systems, so that OHSS can become life-threatening in severe cases.

It is a syndrome of very limited duration, since it disappears with menstruation. But it must be controlled and followed up by transvaginal ultrasound, in case it leads to other more serious complications.

Currently, there are few cases of severe OHSS, since the type of ovarian stimulation protocols used and the analytical and ultrasonographic follow-up of follicular development allow to control the state and response of the ovary to medication and thus avoid the appearance of OHSS .





How can it be avoided?

As we have commented, the SHO appears in the presence of the hCG. Therefore, one way to prevent it is by avoiding the administration of this hormone.

One of the most common measures that can be followed in case of suspecting OHSS is to avoid triggering ovulation with hCG and using another type of medication, such as GnRH analogues.
