Is the ligation of the fallopian tubes reversible?

The reversal of tubal ligation is a surgery to detach and reconstruct the fallopian tubes, again allowing natural pregnancy. Therefore, we can say that tubal ligation surgery is not always definitive.

Below you have an index with all the points that we will discuss in this article.

Is the ligation of the fallopian tubes reversible?

Tubal ligation was considered a permanent contraceptive method that was only recommended to women who were already mothers and were sure of not wanting to have more children. However, the advance of medicine has allowed, in certain cases, tubal recanalization, making this process reversible.

Undo Fallopian Tubal Ligation

The fertilization of the ovum by the spermatozoon takes place in the fallopian tubes. When the tubes are tied, the union of both gametes (ovum and sperm) can not occur and therefore gestation is impeded. If we can open the passage through the tubes, we can look for the pregnancy again.

Is there a possibility of pregnancy after the ligature?

Yes, a woman can get pregnant even if she has done tubal ligation in the past. Approximately 15% of women who have tied tubes want to be mothers again but many are unaware that pregnancy after ligation is possible.

Assisted reproduction is one of the options, although most women prefer to undergo a new operation to reverse the ligature and recover the natural reproductive capacity.

Reversing the tubes does not necessarily imply that they are functional again. There are occasions when the intervention has been successful but the pregnancy does not come.

In addition, we must not forget that the fertility of the woman not only depends on the tubal anatomy, so that, even recovering tubal patency, the probability of pregnancy will depend on other factors such as the ovarian reserve of the woman (quality and quantity of ovules ), the quality of the semen, the qualities of the uterus, etc.

For this reason, it is recommended to perform a fertility study prior to surgery to reattach the tubes.

It is important to remember that the reversal of tubal ligation is a much more complex surgery than the ligature itself and with a much longer duration. While the operation of the ligature lasts about five minutes, the tubal recanalization can take up to two hours and therefore the benefit of the intervention must be carefully evaluated.

Success of the reconstruction of the tubes

The success of surgery to untie the tubes will depend on several factors:

  • Segment size cut or blocked: it is necessary at least 4 cm left to not lose functionality.
  • Age of the woman: in women over 40 years of age, the probability of pregnancy is low.
  • Skill or experience of the microsurgeon
  • The way in which the ligature was made.

In relation to the last factor, we can say that if the ligature was made by cutting the trunk and knotting the remaining ends or if a clip was placed for the block, the reversion will consist simply of eliminating the knotted area or the part with the clip and suturing the extremes. With this, the probability of pregnancy in young women (under 35 years) is up to 70%, although it will depend, as we have said, on the portion eliminated.

If, on the contrary, the ligature was done by eliminating the fimbrias (distal part), the reversion is less effective, being only successful in 20-25% of the cases.

Finally, if the Essure system was used to voluntarily block the fallopian tubes, recovering functionality is more difficult since this system may have severely damaged the tube.
