Pyocytes in the spermogram: leukospermia and its treatment
The leukocytes or pyocytes are usually scarce in the seminal fluid or even absent. However, if there is an abundant number of these cells, it can be an indication that there is an infection. This elevated presence of leukocytes is known as leukospermia, leukocytospermia or pyospermia.
Below you have an index with all the points that we will discuss in this article.

High leukocytes in the seminogram
Leukocytes or white blood cells are cells of the immune system that are found in the blood and their main function is to fight infections.
It is usual to observe some in the seminal samples when performing seminograms. However, when they are at an abnormally high concentration, it can compromise male fertility because it increases oxidative stress and causes a worsening of seminal quality, which may affect sperm concentration.
Therefore, the presence of leukocytes in the semen can cause temporary sterility, but it will rarely be a primary cause of sterility. Therefore, in cases in which leucospermia is detected, a specific treatment is necessary to solve this sterility and prevent it from becoming permanent.
To perform the count of leukocytes in the semengram, the amount of leukocytes per milliliter of semen is calculated. According to the World Health Organization (WHO), the reference value is 1 million leukocytes per ml of semen. Therefore, if this value is exceeded, we can affirm that the male suffers from leucospermia, that is, that the leukocytes are high.
Leukospermia: leukocytes in the semenIn the image we can see what the appearance of a seminal sample with leukocytospermia looks like. Leukocytes are distinguished from spermatozoa because they are larger round cells.

Questions from users
If I have leukocytospermia, what treatment do I have to follow?
By Dr. Carmen Ochoa, gynecologist
The first thing would be to identify the origin of leucospermia. If it is infectious, which is demonstrated by a culture of semen, urine and antibiogram, it will be necessary to put an antibiotic treatment and if it is of inflammatory origin, it will be treated with anti inflammatories.
