Fetal death due to problems in the placenta

Fetal death is a frequent cause of miscarriage that occurs after week 20 of gestation and in which the fetus is born lifeless. These tragic deaths occur in approximately 1 in 160 pregnancies in most cases before it begins. childbirth.

Below you have an index with all the points that we will discuss in this article.

Fetal death due to problems in the placenta

Until now, little was known about this type of death, but a recent study by American researchers and published in the Journal of the American Medical Association, has clarified many of the causes and possible symptoms that cause this type of abortion.

Among the causes pointed out by these researchers, complications during pregnancy such as problems in the placenta and infections, could be the cause of these deaths. Thus, after analyzing the data of 500 pregnant women from five different states it was observed that in 26% of the cases, the fetal deaths were due to problems in the placenta and between 14-19% due to some type of infection suffered during the gestation period.

However, in many cases the cause of the death of the fetus could not be determined since many families did not authorize the investigators to perform the autopsy.

What is it?

Fetal death is one of the most difficult obstetric accidents to face since it is a disenchantment that dispels all expectations forged in the future child.

Both the pregnant woman and the fetus are exposed to a series of risks that threaten their health and lives. Among the likely complications is intrauterine fetal death.

This problem reaches frequencies between 7 to 10 per 1000 live births in most of the American countries. Although some authors define it as "the cessation of fetal life after 20 weeks of gestation and weighing more than 500 grams" there is no internationally accepted definition.

According to the World Health Organization (WHO), fetal death is defined as "the death occurred before the expulsion or complete removal of the mother, the product of conception, whatever the duration of pregnancy. Death is indicated when the fetus does not breathe or does not give evidence of life such as the absence of heartbeats, pulsation of the umbilical cord or voluntary muscle movements.

Main causes of fetal death

Traditionally the cause of fetal death has been differentiated into: maternal causes, fetal cause and placental cause.

Overcoming fetal death

The causes are multiple and can be attributed to diverse origins as environmental, maternal, fetal and ovular, and partly to unknown factors. That is why during pregnancy the fetus can die by:

  • Reduction or suppression of uterine-placental blood perfusion: caused by arterial hypertension, induced or pre-existing to gestation, maternal heart disease, maternal arterial hypotension due to acute anemia or another cause.
  • Reduction or suppression of oxygen supply to the fetus: may be caused by alterations of the syncytrophoblast membrane, infarction and calcifications of the placenta, knots, exaggerated torsion and procidence or prolapse of the umbilical cord, or other factors.
  • Insufficient caloric intake: due to severe maternal malnutrition.
  • Imbalance of the metabolism of carbohydrates and acidosis: for severe or decompensated diabetes.
  • Hyperthermia, bacterial toxins and parasitosis: Serious virosis, bacterial and parasitic infections of the mother, of the ovular membranes, of the fetus and of the placenta intervene.
  • Maternal poisoning: the accidental intake of mercury, lead, DDT, etc.
  • Trauma: can be direct (the fetus) or indirect, through the mother. The most important one is acquiring the direct one due to bad technique in the application of invasive methods such as cordocentesis.
  • Congenital malformations: being incompatible with the fetal growth and development (cardiac, of the encephalon, etc.)
  • Alterations of fetal hemodynamics: as in univitelline twins.
  • Unknown causes: the usual death of the fetus has been denominated to this picture.
