Sperm aspiration by PESAThe technique involves extracting sperm directly from the epididymis, the area of ??the male reproductive system located above the testicle. Here the spermatozoa are stored once produced and complete their maturation.
Thanks to this technique of sperm retrieval, sperm can be frozen for later use or directly performed an ICSI with them.
Surgical intervention will be done in one way or another depending on whether the procedure is possible through the skin (percutaneous) or an incision must be made. Thus, two methods for sperm aspiration are distinguished: PESA and MESA.
In cases in which no sperm can be obtained, a testicular biopsy can be performed to obtain sperm directly from the testicle. Another option would be the donation of semen .
Percutaneous sperm aspiration (PESA)
By means of this method the content of the epididymis is gently and gradually aspirated by means of a butterfly needle. The process takes no more than 10-20 minutes and only local anesthesia is required.
The extracted fluid is then examined under a microscope to see if there are sperm cells and to be able to analyze their mobility and concentration. These, after being processed in the laboratory, can be used to microinject and fertilize the couple's eggs.
Percutaneous recovery is usually the first option to obtain sperm for an ICSI treatment, since it is a simpler and less invasive procedure than microsurgical aspiration.
ICSI with sperm obtained by MESAHowever, not enough sperm are obtained in the event that the sample is to be frozen, so it may be necessary to repeat the procedure.
So that this does not happen, those samples that are going to be frozen are obtained through MESA, which is a more reliable technique and with a higher collection volume.

Microsurgical Sperm Aspiration (MESA)
On the other hand, the MESA method consists of making a 1-2 cm incision in the skin of the scrotum in order to expose a testicle and reach the epididymis.
Once there, the ducts inside the epididymis are examined and opened to extract the fluid from inside. This fluid is then analyzed in the laboratory through a microscope to look for sperm. In case they are and are of good quality, they are aspired for their subsequent freezing and / or use in an ICSI.
Through this procedure more sperm are extracted than in a PESA, and being an extraction with the epididymis open it is possible to reach different areas and see the area with greater clarity.
It has the disadvantage that it is a more complex, more invasive procedure and it is necessary to use general anesthesia for sedation of the patient.
