Puerperal or postpartum fever

Puerperal fever is diagnosed when a temperature higher than 38ºC is detected, on two separate occasions for at least six hours, in the 24 hours after delivery until 6 weeks postpartum.

In the first 24 hours the temperature must be above 38.5 ºC to consider it important.

Puerperal or postpartum fever

It is the third cause of maternal death and is usually in women with a history of urinary tract or vaginal infections, or who suffered premature membrane rupture, prolonged labor or undergoing caesarean sections.

Causes of puerperal fever

It is caused by the lack of hygiene in the care during delivery or the puerperium, or because a part of the placenta has remained in the uterus.

The main causes that can cause puerperal fever are:

  • Puerperal endometriosis: bacterial infection of the endometrial tissue.
  • Surgical wound infection (cesarean section or episiotomy).
  • Puerperal mastitis: infection of the breast parenchyma. Related to breastfeeding.
  • Other systemic infections

Local complications such as pelvic cellulitis, pelvic thrombophlevitis, peritonitis or pelvic abscesses may occur. The causative microorganisms are usually Streptococcus agalactiae, Streptococcus pyogenes or Escherichia coli.

Diagnosis and treatment

For the diagnosis of the origin of the fever, the intrinsic risk factors of the mother and the risk factors related to the delivery must be identified. A gynecological and systemic physical examination is also carried out.

You can perform other complementary tests such as blood tests, blood cultures, endometrial culture, etc.

Antibiotics such as amoxicillin or gentamicin are used to treat febrile symptoms of unknown origin.

If the cause of the fever is determined, an appropriate treatment will be carried out for each pathology.

It is essential to maintain good intimate hygiene during the puerperium to avoid possible infections. It is recommended to wash and disinfect the external genitalia at least 2 or 3 times a day.

On the other hand, if symptoms occur and the infection is not treated, it can cause death. Therefore, it is necessary to go to a hospital center and not self-medicate.
