Intracytoplasmic sperm microinjection (ICSI) is an assisted reproduction technique that is used very frequently instead of conventional in vitro fertilization (IVF).

Indications of ICSI

It is used for the most serious problems of infertility , male infertility being the most common reason for the indication of this technique of assisted reproduction . In certain cases of female infertility and for other causes such as previous failures of IVF, ICSI may also be indicated.

Male infertility

In cases of severe male factor is necessary to perform ICSI to obtain good results. This is usually the case of oligozoospermias, astenozoospermias and severe teratozoospermias, as well as in cryptozoospermias and azoospermias.

The poor quality of the semen

In case of moderate teratozoospermia, the pregnancy rate decreases if a conventional in vitro fertilization is performed, so ICSI should be performed.

In cases of total teratozoospermia or azoospermia, it is essential to perform ICSI if a testicular biopsy or epididymal sperm aspiration is performed to try to locate some sperm or some spermatid with which to fertilize the ovule.

In the case of males who have anejaculation or problems in getting an ejaculate under normal conditions due to retrograde ejaculation or some other pathology, ICSI is also performed, as with semen samples from men with infectious diseases such as HIV or hepatitis.

In addition, ICSI is performed with valuable semen samples such as those of frozen semen from men who have undergone radiotherapy / chemotherapy or from men who have undergone a vasectomy.

Feminine infertility

Although it is more infrequent, ICSI is also indicated in certain cases of female infertility:

  • Obtaining a low number of oocytes in the ovarian puncture
  • Oocytes with thickened zona pellucida
  • Poor oocyte quality

If few oocytes are obtained or if the oocyte quality is low it is necessary to ensure that fertilization occurs, so the usual technique is ICSI.

Injection of the sperm into the ovum

In situations where the zona pellucida of the oocytes is thickened, it will be very difficult for the sperm to cross it by themselves in a conventional in vitro fertilization, so it is necessary to microinject these oocytes.



Indicated by other causes

There are other causes for which ICSI is usually performed. Among the most frequent are:

  • Preimplantation genetic diagnosis (PGD)
  • Failure of fertilization
  • Previous FIV failures
  • Previous failures of artificial insemination
  • Rescue microinjection for non-fecundation after conventional IVF
  • In vitro maturation of oocytes
  • Immunological sterility by anti-sperm antibodies

When a PGD is going to be performed, ICSI tends to be performed to avoid the permanence of granulosa cells stuck to the surface of the ovule, since these could contaminate the blastomere that is extracted in the biopsy and could cause a diagnostic error.
