PICSI (physiological ICSI)

In traditional ICSI sperm selection is performed by observing the embryologist, who observes in the semen sample a spermatozoon with good mobility and selects it to perform the ICSI.

By means of the physiological ICSI, or pICSI, it is intended to find a less subjective system in the spermatozoon selection system, and is performed using a molecule similar to the one that naturally wraps the oocyte.

Indications

This technique of assisted reproduction is quite new, so it is still being studied which patients to indicate, but in a general way the clinics have decided that it is more convenient to use in cases of:

  • Patients with high levels of DNA fragmentation in their sperm.
  • Previous ICSI failure due to poor embryo quality.
  • Cases of repeated abortion suspected to be male, or cause unknown.

Process

The procedure of the pICSI is the same as that of ICSI by changing the part of the selection of the male gamete, so we will only explain this point in particular.

The collection of semen is done normally, and after training the spermatozoa uses a special plate to deposit spermatozoa.

Advantages of PICSI

This plate contains a few drops of a synthetic material very similar to the hyaluronic acid that covers the oocytes naturally. So good quality sperm will be attached to these drops, so that the embryologist can easily identify them and use them to perform the ICSI. Once the spermatozoon is selected, aspirate with the microinjection pipette and the ICSI is performed normally.

Advantage

This method is very useful in cases of patients who have good sperm motility, with high DNA fragmentation, because they help to choose mature and good quality spermatozoa.

These spermatozoa are individually selected and present low fragmentation rates, reducing the possibility that genetic alterations such as aneuploidies exist.

So that the possibility of forming good quality embryos is higher, also increasing pregnancy rates.
